RESERVATION STAGE

1) Proof of income 

✓ latest 3 months payslip 

✓ COEC Certificate of Employment with Compensation 

( if thru online or email, screen shot kung saan naka send )

2) Two valid ID's both spouse if married. 

3) Birth Certificate or/Married Certificate if married. 

CENOMAR ( if applicable )

4) 4 pcs 1X1 ID pictures white background both spouse if married. 

5) BIR TIN  VERIFICATION ( click the link and complete verification )

6 ) PAG IBIG HDMF MEMBERS FINANCING Proof of Pag - ibig Contribution or Print out of ESAV 

( kapag may Pag Ibig Portal, screen shot lahat ng contributions)

7 ) Pag Ibig Loyalty Card ( applicable kapag isa lang Ang valid IDs)

8 ) Consolurized / Notarized (SPA) Special Power of Attorney (pwede ng scanned muna with printed name and signature sa form ng SPA) thru follow Ang original.

9 ) Two Valid IDs of Attorney in Fact / Authorized Person (if applicable)
