3) Two valid ID's both Spouses if Married. (zerox with spicemen signature)
4) Consolurized / Notarized (SPA) Special Power of Attorney
5 ) Two Valid IDs of Attorney in Fact / Authorized Person
6 ) Birth Certificate for Single or / Married Certificate if married
7 ) 4 pcs 1X1 ID pictures white background
8 ) BIR TIN VERIFICATION ( click this link to complete your BIR TIN & screen shot the result)
9 ) for PAG IBIG HDMF MEMBERS FINANCING Proof of Pag - ibig Contribution, Print out of ESAV ( kung may PAG IBIG PORTAL, screen shots lahat ng mga contributions)
10) Pag Ibig Loyalty Card ( applicable kapag Isang valid IDs available)
