1) Two years ITR 

✓ with financial statement duly signed by the accountant 

✓ with Official Receipts proof of payment from the bank 

2) Business 

✓ DTI Registration 

✓ Business Permits 

✓ Mayor's Permit 

✓ picture of the business

3) Two valid ID's both spouse if married.

4) Birth Certificate for single or/ Married Certificate if married

5) 4 pcs 1X1 ID pictures white background 

6) BIR TIN VERIFICATION ( click this link, complete the verification and screen shot the result )

7 ) PAG IBIG HDMF MEMBERS FINANCING Proof of Pag - ibig Contribution, Print out of ESAV. ( kung may PAG IBIG PORTAL, screen shots all contributions available )

8  ) Pag Ibig Loyalty Card

(optional if walang two valid IDs)

9 ) Notarized / Consulorized SPA (Special Power of Attorney) if applicable 

10 ) Two Valid IDs of Attorney in Fact / Authorized Person (if applicable)
